How We Can Help
Once you reach out to connect with a Resource Navigator, we will contact you on a regular, ongoing basis to discuss your individual situation. As we learn more, we will assess unmet needs and help you create an action plan with simple, practical solutions.
Action steps could include connecting with community resources, trying new communication strategies, implementing a home safety change, etc. We will then check in on your progress and discuss next steps.
There is no end to Memory Link services. We're here as long as you want to connect with us.

Who is Memory Link for?
- People living with dementia or experiencing symptoms of memory loss
- Care partners of people living with dementia
No diagnosis required
Memory Link Service Areas
To provide high quality resource navigation, persons living with dementia must live within:
Calumet, Outagamie, Waupaca, or Winnebago counties*
*Care partners can live anywhere as long as their loved one with dementia lives within these counties.
