Fox Valley Memory Project partnering organization, Mosaic Family Health / Fox Valley Family Medicine Residency Program, is a Wisconsin Alzheimer’s Institute affiliated Memory Assessment Center. At the clinic (229 S Morrison Street, Appleton) Dr. Lee Vogel, Executive Director and geriatrician, and staff, along with the resident family practice physicians, complete comprehensive assessments. Services are offered to both patients of Mosaic and those who receive their primary care in other clinics.

A home or clinic visit is scheduled to meet the person and their care partners to begin information gathering. Initial assessments screen for memory impairment, assess overall health, and if warranted, proceed to a more comprehensive evaluation to diagnose the dementia. Both the voice of the person affected, as well as that of the care partner(s), are honored.
Findings of the assessment are discussed with the persons and their loved ones and care partner(s). Education is vital to both understanding and planning for the future. Information is also communicated the primary care physician. Relevant community resource recommendations are also made and assistance with accessing those services is given as needed. The persons and their families also have the ongoing support of the Fox Valley Memory Project, the Mosaic Care Coordinator, as well as other community resources.
